Description
"GUSTIE'S DON'T HAVE TIME FOR THE FLU!"
When: 11 a.m. - 1 p.m. & 4 - 6 p.m. on Wednesday, October 29th, 2008
Cost: $20 Cash/Check or Bill to Insurance
If you would like the vaccine billed to your insurance you will need to BRING YOUR CARD to the Flu Shot Clinic. You will also need to provide the policy holder's name, address, and date of birth.
